29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position: a) Jobsite, 123 Theater Drive, Duncansville, PA: On or before 04/11/13, an employee was climbing the rails of the basket of 400S Sky Trac Aerial Lift to gain access to the roof. The employee was not connected to a restraint device or personal fall arrest system when climbing the rails of the basket exposing the employee to a 27 foot fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite, 123 Theatre Drive, Duncansville, PA: On or before 04/11/13, the employer did not provide training to an employee working from the basket of a 400S Sky Trac Aerial Lift installing a metal roofing system. The employee was not connected to a fall arrest system while climbing the rails of the basket of the 400S Sky Trac Aerial Lift to gain access to the roof and was exposed to a 27 foot fall hazard.
