29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a) Jobsite, South Midway Service Plaza (Renovation), 499 Shady Drive, Bedford, PA: On or before 04/11/13, employees working on a fabricated frame scaffold located at the front of the building were exposed to fall hazards ranging from 10 feet 8 inches to 16 feet.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   a) Jobsite, South Midway Service Plaza (Renovation), 499 Shady Drive, Bedford, PA: On or before 04/11/13, employees were working on levels of the fabricated frame scaffold that were not fully planked.  Employees were exposed to fall hazards up to 16 feet from the openings on the walking/working levels.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, stucture, personnel hoist, or similar surface was not used:   a) Jobsite, South Midway Service Plaza (Renovation), 499 Shady Drive, Bedford, PA: On or before 04/11/13, a Louisville fiberglass extension ladder, 24 foot, model FE 3224, used as part of the scaffold system, did not extended to all of the working/walking platforms accessed by the employees.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a) Jobsite, South Midway Service Plaza (Renovation), 499 Shady Drive, Bedford, PA: On or before 04/11/13, employees were standing on the 8/12 pitch roof of the building installing plywood on the gable end of the building. The employees were exposed to a 17 foot fall hazard.
