29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) 1134 E. 7th Street, Plainfield, NJ: 1. Employee using a cord-and-plug Bosch circular saw was observed performing cutting operations without the use of eye or face protection. 2. Employee was observed utilizing a pneumatic nail gun to install shingles on the rear roof without the use of eye or face protection. On or about April 8, 2013.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 1134 E. 7th Street, Plainfield, NJ: 1. Employee was observed performing roofing operations on a 12/12 pitched roof with an eave height of approximately 24' to the ground below with no fall protection in place, exposing him to fall hazards. 2. Employee was observed performing roofing operations on a 2/12 pitched roof with an eave height of approximately 11' to the deck below with no fall protection in place, exposing him to fall hazards. Condition observed on or about April 8, 2013.
