29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    A.)  Lewis & Hockenberry Inc., dryside; warehouse - On or about, 4/9/13 employees were charging an electric forklift (Caterpillar model# EC30K) on the dryside/warehouse area.  An eyewash unit was not deployed/installed by the battery charger.

29 CFR 1904.29(b)(1):  A Log of recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:   A.)  Lewis and Hockenberry Inc. - The entries in Column F on the OSHA 300 Log for Calendar Year 2013 were not descriptive in that the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  This occurred in case number 1; 1/11/13, and case number 2; 3/5/13.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    A.)  Lewis & Hockenberry Inc. - On or about 4/9/13, the lockout/tagout procedures were missing key elements, which included but were not limited to, magnitudes of energy and steps to release the machinery/equipment from lockout.  Personnel performed servicing/maintenance on machinery/equipment that included but was not limited to the chain feeder, debarker, and  cut off saw.
