Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. a.) Tecnico Corporation: On or about 04/04/13, rubber protective gloves used in testing 450 volts switch boxes were not electrically tested every six months thereby exposed employees to a potential shock hazard.

Structural shop: On or about 04/04/13, the propylene supply line used to cut steel from a bow ramp was not equipped with a flash back protection device thereby exposed employees to potential burns.

29 CFR 1910.253(e)(5)(iv): Hose connections shall be clamped or otherwise securely fastened in a manner that will withstand, without leakage, twice the pressure to which they are normally subjected in service, but in no case less than a pressure of 300 psi (2.04 MPa). Oil-free air or an oil-free inert gas shall be used for the test. a.) Tecnico Corporation: On or about 04/04/13, oxygen and fuel gas hoses were not properly tested after couplings were clamped onto the hoses, in that, tool service air at 127 psi was used to test the hoses for leaks creating a potential fire hazard if hoses rupture.
