29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, 1305 Pittsburgh McKeesport Boulevard, West Mifflin, PA: On or about 04/12/13 employees were removing concrete c-channels and no fall protection systems were in place to protect the employees from the unprotected side or edge exposing them to an approximate 14 foot fall to the floor below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) Jobsite, 1305 Pittsburgh McKeesport Boulevard, West Mifflin, PA: On or about 04/12/13 a 4 ft. Werner step ladder was used for purposes other than which it was designed. The step ladder was in the closed position and was leaning against the parapet wall and was used by the employees to access/egress the roof from the extension ladder erected on the outside wall.
