29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  (a)  Twin Hickory Station near Wick, WV:  A 12-volt battery was connected to the Condensate Stabilizer that was located in a potential flammable atmosphere and the battery was not stored in a sealed container and the battery cables were not connected in a manner that would prevent flammable vapors in the work area from entering into the battery storage case where the battery could become a source of ignition as determined on April 12, 2013.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":    (a)  Twin Hickory Station near Wick, WV:  The gate located on the South side of the fenced-in compound where flammable natural gas condensate liquids were processed was not marked indicating it was an exit nor was it distinguishable from the rest of the chain link fence so that it would be apparent that it could be used for emergency egress from the site during a fire or other emergency as determined on April 12, 2013.    (b)  Twin Hickory Station near Wick, WV:  The gate located on the West side of the fenced-in compound where flammable natural gas condensate liquids were processed was not marked indicating it was an exit nor was it distinguishable from the rest of the chain link fence so that it would be apparent that it could be used for emergency egress from the site during a fire or other emergency as determined on April 12, 2013.    ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical   (a)  Twin Hickory Station near Wick, WV:  The two-phase separator, serial number NIP-3167, that contained flammable natural gas and natural gas condensate liquids was not labeled indicating its contents and associated hazard warnings as determined on April 12, 2013.   (b)  Twin Hickory Station near Wick, WV:  The four Storage Tank Trailers that received the flammable condensate liquids from the Condensate Stabilizer did not have labels indicating their contents and the associated hazard warnings as determined on April 12, 2013.   ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s):  (a)    Twin Hickory Station near Wick, WV:  The single sign posted at the site front entrance that stated "Caution: Natural Gas" did not warn people approaching the site of the potential for a flammable environment.  On April 11, 2013 flammable vapors were vented into the work area during a gas line pigging operation as determined on April 12, 2013.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.
