Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23.   a) On or about April 10, 2013 a thirteen riser stairway that provide access to the Lost and Weigh feeder deck was not equipped with a standard midrail.   b) On or about April 10, 2013 a sixteen riser stairway that provide access tho the Mixer/ Pallet Stirrer deck was not equipped with a standard midrail.  c) On or about April 10, 2013 a twelve riser stairway that provide access to the Mixer deck was not equipped with a standard midrail.

29 CFR 1910.134(h)(2)(i): All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve.  On or about April 10, 2013 employees wearing the 3M 2091 (P-100 NIOSH respirators) was not stored in a bag or other compartments to eliminate contamination.
