29 CFR 1910.305(j)(4)(v): The disconnecting means shall be readily accessible. If more than one disconnect is provided for the same equipment, only one need be readily accessible. (a) Northeast corner of Storage Area above Truck Shop Offices - On or about April 11, 2013 employees were exposed to electric shock and/or burns due to not being able to gain ready access to disconnect breakers for lights, welders, exhaust fans, an air compressor and a transformer because the front door to the 480 Volt AC Square D disconnect panel had been sealed shut with a sheet-metal screw through the panel door.

29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent. (a) Truck Shop - On or about April 11, 2013 employees working in the offices under an overhead storage area were exposed to loads dropping through the ceiling due to the absence of an affixed sign being posted with the approved floor load-rating for the overhead storage area.
