Employee was observed working on roof top at the edge without any means of fall protection, thereby was exposed to an approximately 12 foot fall hazard to the ground. Violation observed on or about 04/10/2013.

Portable ladder did not go 3 feet above the upper landing surface, thereby was exposed to fall hazard to the next lower level. Violation observed on or about 04/10/2013.
