On or about March 27, 2013, at 30520 Tracy Rd. Walbridge, Ohio, the employer did not ensure employees were protected from falls hazards while loading a semi-trailer. The forklift operator was still loading the semi-trailer when the yard operator pulled the semi-trailer away from the dock which resulted in the forklift and the powered industrial truck operator falling to the ground below.

Walbridge, OH 43465; On or about April 15, 2013, the employer did not assure the 90752 Ottawa Switcher Truck was taken out of service until it was restored to safe operating condition. Employees are required to use the switcher truck to move trailers throughout the yard and are exposed to struck by hazards from a broken window and not hooking up brake lights. The rear door is also defective in that it is unable to be closed.
