29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet:  a)  Meadow Lane, W.B. Homes, Sellersville - On or about April 4, 2013, employees are using two extension ladders to access the roof of a residential home project each of which did not extend 3 feet above the upper ladder surface nor were tied off to prevent displacement under load.
