29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet above or below point of access, was not provided with a ladder or direct access from another structure or other equivalent safe access during use:   a- New Single-family Home, Corner of Hazel Ave. and W. Broad St., Westfield, NJ - Access/egress from a pump jack scaffold was via an open window in excess of 14 inches from the wall.    Condition noted on or about 4/10/13.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a- New Home Construction, Corner of Hazel Ave. and West Broad St., Westfield, NJ - Employee(s) working from a pump jack scaffold, approximately 18 ft. from the ground, was not provided adequate fall protection, in that mid rails and side rails were not installed to provide the employee(s) from a fall hazard.  Condition noted on or about 4/10/13.
