29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.  (a). Eastside of shop - On or about April 11, 2013, the blowers used to circulate air in the shop area had unguarded blades.

29 CFR 1910.213(h)(1): The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed.  (a). Westside of shop, radial arm saw - On or about April 11, 2013, the lower portion of the Dewalt radial arm saw blades were not guarded.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  (a). Throughout shop - On or about April 11, 2013, the pulleys for the blowers, air compressor and pallet notcher were not guarded.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures.  (a). Throughout shop - On or about April 11, 2013, the equipment circuit breakers and an electrical receptacle had exposed electrical parts.  The wiring connected to the top and bottom of the circuit breakers were not covered. The circuit breakers were attached to structural poles throughout the shop.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.  (a). Throughout Shop - On or about April 11, 2013 wiring entering electrical boxes were not protected from abrasion and the openings were not effectively closed.
