29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about April 12, 2013, flexible cords were not connected to devices and fitting so that tension would not be transmitted to joints or terminal screws. a) The insultation on the flexible power cord coming from the the disconnect box and leading to the hand polisher was not connected, exposing the internal wiring. b) The insulation on the flexible power cord was pulled away from the cord cap, exposing the internal wiring.
