Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fire, explosion and corrosive hazards:      Warehouse Facility:  On, or about, April 16, 2013, employees were exposed to potential releases of anhydrous ammonia from the closed loop refrigeration system installed in the facility in that,    A.  Engine Room:  An adequate source of make-up air had not been provided for the ventilation system.  Also, the door providing access to/egress from the room was not a tight fitting, self-closing fire door.    B.  Pump Package Room:  An adequate source of make-up air had not been provided for the ventilation system.  No empirical data was available to show that leaving the door open to the room would be sufficient.  Also, the room was required to be provided with a tight fitting, self-closing fire door in order to separate the equipment in the room from the rest of facility.

29 CFR 1910.36(b)(1):  At least two exit routes were not available in a workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section:    Engine Room:  On, or about, April 16, 2013, only one exit was provided from the Engine Room.  The length of the room exceeded the common path of travel exemptions listed in National Fire Protection Association Standard NFPA 101 Life Safety Code.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  Rear Rail Dock Corridor:  On, or about, April 22, 2013, directional signs were not provided to direct employees to exit doors after the passed through the main corridor overhead door pass-through.  Once through the overhead door, there were no signs to indicate the location of the exit doors.
