29 CFR 1926.501(b)(13):  Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 926.502:        This violation most recently occurred on April 16, 2013, at a workplace located at 628 Matthews Avenue, Richardson, TX 75081; where at least one (1) employee was observed working framing a multi-story, multi-family residence, not protected from falling an approximate eleven (11) foot, two (2) inches to the ground level below.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    This violation most recently occurred on April 16, 2013, at a workplace located at 628 Matthews Avenue, Richardson, TX 75081; where two (2) employees were working framing a multi-story, multi-family residence, climbed the stud walls and bracing to access elevated work areas.
