29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:        a) On or about 4/16/13 on the first floor of Medical/Dental Office building under construction at 711 Youngs Rd., Tonawanda, NY: A Portable Work Light was connected to 120 volt-ac power via an extension cord that was plugged into a wall receptacle inside the building. No GFCI or assured equipment grounding program was being used.        NO ABATEMENT CERTIFICATION REQUIRED
