29 CFR 1910.95(i)(4):     The employer did not provide training in the use and care of all hearing protectors provided to employees:    (a) Pickens Road, Nitro, WV:  An employee was exposed to occupational noise at 66% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 405 minute sampling period on 04/17/2013.  For a full eight hour shift this would be equivalent to an average sound level of 87.0 dBA. The employer required and provided hearing protection for this employee and other employees but did not train employees on the use and care of hearing protectors as determined on 04/17/2013.     ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
