29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a)  Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 21.6 feet above the ground.     b) Employees engaged in roofing activity were not protected from falling through skylights, approximately 10 feet above the level below.      No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:      a)  Employer did not provide training to each employee in order to recognize hazards associated with working at an elevation and to understand procedures to control or minimize those hazards.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    a) Employees were using an aluminum extension ladder that contained structural defects, i.e. bent rungs.    No abatement certification or documentation is required for this item.
