29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a) 5110 Market St, Scaffold - The outrigger section of the tubular welded frame scaffold was not fully decked.  Employees installing mesh for stucco were exposed to a fall hazard of approximately twenty-six (26) feet.  Observed on or about April 16, 2013.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:    a)  5110 Market Street, Scaffold - A ladder or other safe means of access was not provided to employees performing stucco operations on a tubular welded frame scaffold.  Employees were using the side frame as a means of access/egress, thus being exposed to fall hazards up to twenty-six (26) feet above ground level.  Observed on or about April 16, 2013.

Employees were working on the 4th level outrigger section.  Observed on or about April 16, 2013.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would  automatically square and level:    a)  5110 Market Street, Scaffold - The base frames of the tubular welded frame scaffold did not have any cross or diagonal braces to secure vertical members together laterally.  This condition exposed employees to a collapse hazard while performing stucco operations.  Observed on or about April 16, 2013.
