Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 15, 2013- at the above addressed jobsite, an employee performing carpentry operations on the roof deck, approximately fourteen (14) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about April 15, 2013 - at the above addressed jobsite, an employee conducting framing operations from a corrugated floor deck, approximately thirteen (13)) feet above the ground, was exposed to falls to the inside of the building, created by the presence of an unprotected floor hole opening.
