On 3/11/2013, an employee working at Schwoeppe Machine & Tool located at 16240 Highway O, Marthasville, MO, 63357, had his hand crushed while cleaning an CNC Assembly and Test Machine, serial # 5858DM.  The CNC machine was not locked out/tagged out while the employee was cleaning it and while another employee was programming the programmable logic controller.  As a result, the machined cycled and crushed the employees hand.

The individual procedures are missing a step requiring the authorized employee to attempt to activate or test the machine after the machine has been LOTO to ensure the machine is isolated from hazardous energy.
