29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.         On 3/11/2013, an employee working at Schwoeppe Machine & Tool located at 16240 Highway O, Marthasville, MO, 63357, had his hand crushed while cleaning an CNC Assembly and Test Machine, serial # 5858DM.  The CNC machine was not locked out/tagged out while the employee was cleaning it and while another employee was programming the programmable logic controller.  As a result, the machined cycled and crushed the employees hand.

29 CFR 1910.147(c)(4)(ii)(D):    The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  On 4/17/2013, employees working at Schwoeppe Machine & Tool located at 16240 Highway O, Marthasville, MO, 63357, were not provided proper LOTO procedures for twenty-nine machines.  The individual procedures are missing a step requiring the authorized employee to attempt to activate or test the machine after the machine has been LOTO to ensure the machine is isolated from hazardous energy.

The certification did not identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  On 4/17/2013, Schwoeppe Machine & Tool located at 16240 Highway O, Marthasville, MO, 63357, did not certify that periodic inspections had been performed for the LOTO program.  There was no certification completed for employees at the site.

29 CFR 1910.213(i)(1):    All portions of the saw blade were not enclosed or guarded, except for the working portion of the blade between the bottom of the guide rolls and the table.       On 4/17/2013, Schwoeppe Machine & Tool located at 16240 Highway O, Marthasville, MO, 63357, did not have all portions of the blade enclosed or guarded for the band saw, serial # 1380332.  Approximately 10 of the blade was unguarded beneath the table near the saw speed adjustments.
