Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:   (a)  On or about April 17, 2013, approximately three employees were exposed to fall hazards where the employer failed to properly install the emergency egress or Geronimo line.  The ground anchor point or dead man was measured at approximately 70 feet and the upper anchor point was at approximately 50 feet from the ground, creating an angle in excess of 45 degrees.  Located at Bandit 1-29, approximately one mile south of the intersection of Highway 23 and 82nd Avenue NW, near New Town, ND.   Note: Among other means, a feasible and acceptable means of abatement would be for the employer to correctly install a safety slide or Geronimo line in accordance with the best established industry practices.  The manufacturer's guidance, which identifies the requirements for the Geronimo line being anchored to the ground at a distance of twice the working platform's height, and references established by the (API) American Petroleum Institute Recommended Practice 54, Third Edition "Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations", section 6.10, "AUXILIARY ESCAPE", subsection 6.10.1, "It is recommended that the ground anchor point of the escape line should be located a minimum lateral distance from the derrick or mast equal to two times the height of the work platform.  If the rig configuration or location configuration will not permit use of the escape system, an alternate means of fast emergency exit from the derrickman's working platform to a safe place should be provided."
