The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a)     On or about April 17, 2013; at the center of the jobsite, the employer allowed employees to utilize an extension cord that was missing a ground pin.  The extension cord was used to deliver power from a temporary power distribution panel to a portable flood light.  Employees used the flood light to illuminate the work area.  The missing ground pin prevented the extension cord from having a continuous path to ground.
