29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Facility- 4 Race Street, Washington, PA: On or about April 17, 2013, each employee operating a powered industrial truck was not trained on their forklifts. Brookman Iron & Metals CO. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 315562603, citation number 1, item number 4 and was affirmed as a final order on May 11, 2012, with respect to a workplace located at 4 Race Street, Washington, PA.

Brookman Iron & Metal CO. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.253(b)(2)(iv), which was contained in OSHA inspection number 315562603, citation number 1, item number 6b and was affirmed as a final order on May, 11, 2012, with respect to a workplace located at 4 Race Street, Washington, PA.
