29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 300 Kenneth Avenue, Johnstown, PA:  On or about April 17, 2013, an employee was working on a residential roof and was not protected from falling.  The fall hazard was about 15 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Jobsite, 300 Kenneth Street, Johnstown, PA:  On or about April 17, 2013, employees were using a 16 foot aluminum extension ladder to access the roof and the ladder was only extended approximately 18 inches above the roof.  The fall hazard to the employee was about 10 feet to the ground below.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a)  Jobsite, 300 Kenneth Avenue, Johnstown, PA:  On or about April, 17, 2013, employees were using a 16 foot Werner extension ladder and did not perform the periodic inspection of it and were using it with a damaged rung that was starting to pull away from the siderail.
