On or about April 16, 2013 - at the above addressed jobsite, a section of the guardrail system erected around the temporary ladder access opening on the 4th, 5th and 6th floors was thirty three (33) inches in height, thereby exposing the employees to fall hazards of approximately eight (8) feet to the lower level.
