29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a.  The point of operation was not adequately guarded as there was a 3/4 inch opening between the blades on the Accur Shear 127.  b.  The point of operation was not adequately guarded on the Accurpress 115 as there was a 2 3/4 inch opening between the dies.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
