29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided:       (a) Efren Ortiz dba Ortiz Roofing @ 916 44th Ave Ct., Greeley, CO 80634: On and before April 12th, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected edges of a roof on a multi-family home. Employees were not utilizing a fall protection system. Employees were exposed to fall hazards of approximately 18'.    Efren Ortiz dba Ortiz Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 313413080, citation number 1, item number 1b, and was affirmed as a final order on 11/9/2009, with respect to a workplace located at 1001-1007 48th Avenue, Greeley, CO 80634  .

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder.      (a) Efren Oritz dba Ortiz Roofing @ 916 44th Ave Ct., Greeley, CO 80634: On and before April 12th, 2013, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  Employees were using a portable extension-ladder for access to the roof. Employees were exposed to a fall of approximately 18' while attempting to use the ladder.
