29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. (a) Jobsite - On or about April 17, 2013 an employee was not wearing a hard hat while working under scaffolding. The employee was exposed to being struck-by a pneumatic nail gun and tools being used above.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. (a) Jobsite - On or about April 17, 2013 employee was installing vinyl siding using a ladder scaffold without fall protection. The employee was exposed to a fall height of 12 feet 04 inches.
