29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) On or about March 28, 2013;west-side 6th floor, employees were operating a table saw which was not equipped with an anti-restart/magnetic switch to prevent it from accidental start up if the power was lost and re-engaged. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. a) On or about March 28, 2013; west-side 6th floor, employees were using a ground fault circuit interrupter that was damaged and thus would not protect them from electrical shock in the event that the portable electrical tools used in the process became damaged.
