29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Throughout the facility - the employer did not establish machine specific procedures for the control of potentially hazardous energy sources when employees were engaged in machine servicing and maintenance. Employees perform maintenance on equipment, such as but not limited to, rollers, drums, extruders, and blowers, without machine specific procedures in place to ensure that all energy sources were de-energized and isolated, exposing employees to electric shock, electrocution, lacerations, and amputation.
