29 CFR 1910.147(c)(4)(ii): The employer did not develop and document energy control procedures that clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about April 18, 2013 employees were exposed to the unexpected energization of a robotic welder when performing service and maintenance activities because an energy control procedure had not been developed to address the energy sources used by the machine.
