29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 2091-2099 McKelvey Road, Maryland Heights, MO 63043, at this work site, workers were observed on top of a 3-story apartment building, 4:12 pitch roof, actively install black felt paper, approximately 25 feet 9 inches eave to ground level and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to fall hazard.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, a grasping device was not provided to assist employees in mounting and dismounting the ladder:  JOB SITE: 2091-2099 McKelvey Road, Maryland Heights, MO 63043, at this work site, workers were observed using a Louisville Aluminum extension ladder on the west end of the apartment to gain access on and off the roof, the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed nor was it secured to prevent from shifting or displacement.  The workers were exposed to fall hazard.
