29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents  On or about 15 April, 2013 at the job site located at 1701 Club Manor Drive, Maumelle, Arkansas the employer did not ensure the employee used protective eye wear while operating the Dewalt 7-1/4 inch circular saw to cut plywood sheets.  The employee on the roof also used a nail gun without the benefit of protective eye wear.  As a result the employees were exposed to damage to the eyes.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use.   On or about 15 April 2013, at the job site located at 1701 Club Manor Drive in Maumelle AR, the employer did not ensure the employee used a Square Shooter lift model 5636 Turbo to lift personnel and to and from the roof.  The shop made lift basket was not approved by the lift manufacturer and was not secured to the lift when employee made the ascent to the roof.  As a result employees were exposed to fall of greater than 15 feet to the ground below.
