29 CFR 1910.147(c)(4)(ii)(A): The employer procedures for the control of potentially hazardous did not include specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about December 19, 2011, and at times prior thereto, at the Central Mallet Unit #371, the Employer did not establish procedures for maintaining well control while collecting well samples. This condition exposed employees to struck-by and burn hazards.
