29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which were supplied and securely affixed by the owner of the building, or the duly authorized agent, in a conspicuous place in each space to which they related:  (a)  The upstairs upper level did not have a load rating marked of the approved load exposing employees to struck-by hazards  from floor collapse.  (b)  The upstairs lower level did not have a load rating marked of the approved load exposing employees to struck-by hazards  from floor collapse.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   (a)  Employees were not utilizing eye protection or face shields when adding water to the acid battery of the Nissan electric forklift.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   (a)  The employer had not accomplished a periodic inspection of the lockout / tagout procedures.  Accomplishing a review of lockout tagout procedures ensures continuity of procedures and proper application.  Inadequate review of changes to equipment and machinery can lead to improper lockout.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     (a) By the side dock door, a Nissan Electric Forklift on a weekly basis would have water added to the battery.  Employees were exposed to corrosive material and not provided with a suitable facility to flush the eyes.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  New employees were not provided with training on the chemicals such as but not limited to:  F-364, Liquid Chlorinated Alkaline Cleaner and F-126, Low Foam Alkaline Cleaner at the time of their initial assignment to inform them of the potential hazards.
