On or about and times prior to 19 June 2013, the employer did not ensure employees unclogging drains lines on roofs at heights 4 feet and greater above a lower surface were provided fall protection by the use of guardrail systems, safety net systems, or personal fall arrest systems , thus exposing employees to a fall hazard.

29 CFR 1910.253(b)(3)(ii): Acetylene cylinders shall be stored valve end up. a) Vehicle V-34- On or about and times prior to 20 June 2013, there was a 35 cubic feet Airgas acetylene cylinder that was no stored with the valve end up. The cylinder was laying on its side in the rear of the vehicle, thus exposing employees to fire, eye, skin and inhalation hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a). Vehicle V-34- On or about and times prior to 20 June 2013, there was no strain relief on a 50 foot yellow flexible electrical extension cord. The outer jacket was pulled back from the male plug end revealing internal wiring, thus exposing employees to electrical hazards.

29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles may not be connected or altered in a manner which would prevent proper continuity of the equipment grounding conductor at the point where plugs are attached to receptacles. Additionally, these devices may not be altered to allow the grounding pole of a plug to be inserted into slots intended for connection to the current-carrying conductors. a). Vehicle V-34- On or about and times prior to 19 June 2013, there was a 50 foot yellow flexible electrical extension cord that was altered in which the ground pin was removed from the male plug end, thus exposing employees to an electrical shock hazard. b). Vehicle V-34- On or about and times prior to 19 June 2013, there was a 100 foot orange and black flexible electrical extension cord that was altered in which the ground pin was removed from the male plug end, thus exposing employees to an electrical shock hazard.
