Energized circuit breakers and electrical parts were partially exposed and partially covered with cardboard paper. On or about 04/15/13. 29 CFR 1926.554(a)(2): Supporting structure to which overhead host was attached did not have a safe working load equal to that of the hoist.     a) Behind the house, south side:     Employees were operating an overhead hoist motor to hoist a single-point suspension scaffold and a section of metal ventilation duct. The overhead host was attached to a roof brick chimney, which was the supporting structure for the hoist. The chimney did not have a safe working load equal to that of the hoist, and was overloaded and collapsed into brick pieces in an accident. On or about 04/15/13.     NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM. 29 CFR 1926.1053(a)(9): The side rails of ladders were spliced and the strength of the resulting side rail was not at least equivalent to a one-piece side rail made of the same material.     a) 3rd floor:     Employees used a portable extension ladder for access to the roof to anchor a suspension scaffold and a hoist system. The ladder was made of aluminum and was spliced with extension of both side rails and an additional rung made of wood. The strength of the resulting side rails was not at least equivalent to a one-piece side rail made of the same material. On or about 04/15/13.
