B. B.  Trash Compactor, Outside of Building: On, or about, April 26, 2013, adequate safeguarding had not been provided on the All Waste trash compactor.  The compactor had not been provided with either an interlocked access door, positive pressure control or loading chute installed at the opening to prevent access to the ram area.
