29 CFR 1926.451(c)(1)(i): Guys, ties, and braces were not installed at locations where horizontal members support both inner and outer legs. a) Jon Schmidt Masonry, Inc., 104th & Federal, Federal Heights, CO 80260: On and before April 19, 2013 the exposing employer failed to ensure that a cross brace was installed on the lower end frame of a scaffold. This condition exposed employees to fall hazards.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows: a) Jon Schmidt Masonry, Inc., 104th & Federal, Federal Heights, CO 80260: On and before April 19, 2013, the exposing employer failed to ensure that all working scaffold platforms were fully planked. The scaffold on lower working level was planked with one 9 inch wide plank. This condition epxosed employees to fall hazards.
