29 CFR 1926.200(g)(1): The employer did not provide the requisite advance warning of construction work being conducted on the traveled way: (a) Oldcastle SW Group, Inc. DBA United Companies at 28 ¼ RD. between Patterson Rd. and Orchard Ave., Grand Junction, CO 81506: On April 11, 2013, the controlling employer did not ensure the subcontractor providing traffic control had at least one advance warning sign to warn road users of a lane closure on 28 ¼ Rd. heading southbound from Patterson Rd. This condition exposed employees in the work zone to a struck-by hazard. Abatement Note: Among other methods, a feasible and acceptable abatement method to correct this hazard is to ensure proper placement of the advance warning sign. This abatement method is depicted in Part VI of the Manual on Uniform Traffic Control Devices (MUTCD) Millennium Edition in Figure 6H-30, Interior Lane Closure on Multilane Street (TA-30).

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD): (a) Oldcastle SW Group, Inc. DBA United Companies at 28 ¼ RD. between Patterson Rd. and Orchard Ave., Grand Junction, CO 81506: On April 11, 2013, the controlling employer did not ensure the subcontractor providing traffic control followed the MUTCD to provide proper signs and devices with the required taper length for the lane closure on 28 ¼ Rd. heading southbound from Patterson Rd. This condition exposed employees in the work zone to a struck-by hazard. Abatement Note: Among other methods, a feasible and acceptable abatement method to correct this hazard is to ensure proper placement of the advance warning sign. This abatement method is depicted in the Manual on Uniform Traffic Control Devices (MUTCD) Millennium Edition in Figure 6H-30, Interior Lane Closure on Multilane Street (TA-30). Abatement Note: Among other methods, a feasible and acceptable abatement method to correct this hazard is to ensure proper placement of the advance warning sign. DBA United Companies at 28 ¼ RD. between Patterson Rd. and Orchard Ave., Grand Junction, CO 81506: On April 11, 2013, the controlling employer did not ensure the subcontractor providing traffic control had followed the MUTCD to instruct flaggers to use proper hand-signaling devices to direct traffic. Employees were directing traffic using hand signals without the aid of paddles and/or flags. This condition exposed employees in the work zone to a struck-by hazard. Abatement Note: Among other methods, a feasible and acceptable abatement method to correct this hazard is to ensure the correct hand-signaling devices and procedures are used. This abatement method is depicted in the Manual on Uniform Traffic Control Devices (MUTCD) Millennium Edition in Section 6E.03 Hand-Signaling Devices.
