29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length extended over its support more than 18 inches (46 cm),and it was not designed and/or installed so that the cantilevered portion of the platform is able to support employees without tipping, or had guardrails which block employee access to the cantilevered end:     (a) 986 Ashby State Rd Fitchburg, MA: On or about April 10, 2013, A Pump jack scaffold set up with a platform greater than 10 feet in length had a cantilevered portion that exceeded 18 inches in length and employees had access and reason to use the cantilevered section of the scaffold which was more than 20 FT above the ground below.       Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  (a) 986 Ashby State Rd Fitchburg, MA: On or about April 10, employees were observed working on a pump jack scaffold more than 20 FT above the ground below without proper fall protection which is not in accordance with this subpart:  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
