Corry Contract, Inc. - On or about 4/18/13, employees were performing servicing and maintenance on machinery including, but not limited to, the Bliss Press brake (#52-150), shears, mechanical power presses, and the Cincinnati Laser (#P14).  The servicing and maintenance activities on the equipment included, but was not limited to, repairs and preventative maintenance. The lockout/tagout procedures for the machinery were missing key elements, including, but not limited to, energy magnitudes, steps on how to deal with stored energy, specific language/techniques to verify deenergization, and steps for the release of lockout.
