29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Corry Contract, Inc. - On or about 4/18/13, employees were performing servicing and maintenance on machinery including, but not limited to, the Bliss Press brake (#52-150), shears, mechanical power presses, and the Cincinnati Laser (#P14). The servicing and maintenance activities on the equipment included, but was not limited to, repairs and preventative maintenance. The lockout/tagout procedures for the machinery were missing key elements, including, but not limited to, energy magnitudes, steps on how to deal with stored energy, specific language/techniques to verify deenergization, and steps for the release of lockout.
