29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about 4/19/2013, at the above addressed jobsite, an employee was engaged in palm tree trimming work from an aerial lift at approximately 14 feet above ground level without a means of fall protection.
