29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde:     a) Hair Salon Body & Soul/Kent Pl., Blvd, Summit, NJ - Salon:  The employer did not monitor employees  who conduct hair smoothing treatments to determine their exposure to formaldehyde. Products used in the salon which contain formaldehyde include, but are not limited to:   -Acai Professional Smoothing Solution contains 6.6520% formaldehyde,  -Keratin Complex Smoothing Therapy Natural Keratin contains 1.8310% formaldehyde,   -Acai Professional Anti-Residue Shampoo contains 0.2810% formaldehyde.     The formaldehyde content is higher than 0.1 % and the salon does not follow the formaldehyde standard requirements.        Condition observed on or about April 19, 2013.

29 CFR 1910.1048(h)(1)(i):  All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation:  a) Hair Salon Body and Soul/Kent Pl. Blvd, Summit, NJ - Salon: Employees' eyes and face were not protected with personal protective equipment (PPE) such as safety glasses and goggles, during transfer and application of hair smoothing products which contain formaldehyde. Employees use Products used at the salon include: - Acai Professional Smoothing Solution which contains 6.6520% formaldehyde - Keratin Complex Smoothing Therapy Natural Keratin which contains 1.8310% formaldehyde     Condition observed on or about April 19, 2013.

29 CFR 1910.1048(i)(3): When there was a possibility that an employee's eyes could be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer did not provide acceptable eyewash facilities within the immediate work area for emergency use:  a) Hair Salon Body & Soul/10 Kent Pl. Blvd, Summit, NJ - Salon: The employer did not provide acceptable eyewash facilities within the immediate work area for emergency use. Employees perform hair smoothing treatments and use products that contain greater than 0.1% formaldehyde. The formaldehyde content of the hair smoothing products used in the salon is as follows: - Keratin Complex Smoothing Therapy Natural Keratin contains 1.8310% formaldehyde,  - Acai Professional Anti-Residue Shampoo contains 0.2810% formaldehyde.   Condition observed on/or about April 19, 2013.

Blvd, Summit, NJ - Salon: A written Hazard Communication Program was not provided for employees using products that contain formaldehyde.
