Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to a flash fire hazard: On April 16, 2013, University 42 Lease, Well #1L, the employer did not ensure precautions were taken to maintain well Control. Employees on site did not secure the blowout preventer (BOP) prior to beginning the hot watering operation, exposing employees to a flash fire hazard.
