29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  69th Water Street, Newton, NJ.  Employees were working on a tubular frame scaffolds that were approximately 8 foot in height were not fully planked, thereby exposing employees to fall hazard.                                    Violation observed on or about 04/16/2013.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  69th Water Street, Newton, NJ.  Inspection was not performed of a tubular framed scaffold being utilized by employees before work started.                                      Violation observed on or about 04/16/2013.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a)  69th Water Street, Newton, NJ.  Employee was observed working from a low-slope roof was not protected from falls thereby exposing the employee to a fall of 8 feet to the ground.
