29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Intersection of Warren Street and South Main Street, Manchester, CT 06040, on or about 04/18/2013: The employee was using an aluminum extension ladder to egress from a 16 foot deep trench and the side rails did not extend at least 3 feet above the upper surface. Ludlow was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 313515926, citation number 01, item number 005 and was affirmed as a final order on 12/14/2009, with respect to a workplace located at Weed Road(Behind SHFD), Bethel, CT 06801.

29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads : Intersection of Warren Street and South Main Street, Manchester, CT 06040, on or about 04/18/2013: A trench box had been installed in an excavation and was not secured in a manner to prevent hazardous movement and/or displacement while an employee was working within the box. Ludlow Construction Company Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(g)(1)(ii), which was contained in OSHA inspection number 313515926, citation number 01, item number 003 and was affirmed as a final order on 12/14/2009, with respect to a workplace located at Weed Road (Behind SHFD), Bethel, CT 06801.

29 CFR 1926.652(g)(2): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of a shield system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the shield: Intersection of Warren Street and South Main Street, Manchester, CT 06040, on or about 04/18/2013: The employee was allowed to work in a 16 foot trench where there was approximately 8 feet of unprotected space between the bottom of the trench and the bottom of the trench box. In addition the employee was also working near the steel plate that was placed as a shield from back fill material near the trench box spreaders and the space from the bottom of the steel plate to bottom of the trench was 5 feet. Ludlow Construction Company Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(g)(2), which was contained in OSHA inspection number 313515926, citation number 01, item number 004 and was affirmed as a final order on 12/14/2009, with respect to a workplace located at Weed Road (Behind SHFD)
